What are Transition Services?
Transition Services means a coordinated set of activities for a young adult with a disability that:- Is designed within a results-orientated process that is focused on improving the academic and functional achievement of the young adult with a disability to facilitate movement from school to post-school school activities, including post-secondary education, vocational education, integrated employment, continuing and adult education, adult services, independent living or community participation.
- Is based on the individual’s needs, taking into account the child’s strengths, preferences, and interests.
Illinois Transition Requirements:
An eligible student who requires continued public school educational experience to facilitate his/her integration into society shall be eligible for such services through age 21, inclusive (i.e. through the day before the student’s 22nd birthday).23 IAC 226.50(c)(1)
Students who have participated in a graduation ceremony but have not been awarded regular high school diplomas continue to be eligible to receive FAPE through age 21, inclusive23 IAC 226.50(c)(4)
