In compliance with Care of Students with Diabetes Act, it is imperative to inform the Health Office if your student has diabetes.  It is the obligation of the parent/guardian to submit the Diabetes Care Plan to the district and to sign an authorization, release and acknowledgement form, which will hold as the basis of the Diabetes Care Plan to seve as a section 504 plan or as part of an IEP.  Proper protocol for staff training is implemented in the beginning of each school year to school staff.
